DENTAL INSURANCE PLAN and COVERAGE at DM Family Dentistry in Casa Grande

Most Dental Insurance plans and Companies accepted:
Dental insurance plan cover between 50% - 100% of the cost depending on treatment and insurance plan in effect. As a courtesy we will file claims for you with your Dental Insurance Company and ask for you to pay your estimated co-payment. Please understand that this is only an estimate and is based on the information available to us.

Our Dental office in Casa Grande will accept assignment of dental insurance benefits under the following conditions:
The information MUST be provided at the first visit for verification of insurance benefits: Subscriber birth date and social security number, insurance carrier billing address, phone number and group number. After verification of insurance benefits, we will accept assignment of insurance benefits as partial payment of the total charge. You will be asked to pay your dental insurance company deductible and estimated co-payment. It is your responsibility to follow up with your Dental insurance Company if you do not see a payment from them within 30 days.
